• Prescribed Application Form;
• An updated inventory of local housing backlog;
• Municipal/City Council Resolution for the creation of a Local Housing Board (LHB) with equitable multi sectoral
representation and must have a NGO/PO representative involved in housing and urban development unless stated by law;
• Compilation of minutes of meetings of the LHB;
• An updated Comprehensive Land Use Plan (CLUP) and Comprehensive Shelter Plan (CSP) approved by their respective
LHB (Optional);
• Municipal/City Council approved organizational structure and personnel complement of the unit / department in-charge of
social housing or other housing programs;
• Municipal/City Council ratified AIP indicating the prioritized LCMP project/s for the year and the approved regular budget
allocation for housing (Optional);
• COA Audited Financial Statements for the past three (3) years; and
• Municipal/City Council Resolution authorizing the Local Chief Executive to assign the IRA or other forms of acceptable
securities/guaranty, specifying the amount and period of assignment, in favor of SHFC as well as sign, negotiate and
transact with SHFC and other LCMP partners to fully operationalize and implement LCMP in the locality.
I. APPLICATION FOR OCL
The partner-LGU shall submit to SHFC the following documents in two (2) sets, to wit:
1. Prescribed Application Form;
2. City Council ratified AIP indicating the prioritized LCMP project/s for the year and the approved regular budget
allocation for housing;
3. List of Household Beneficiaries (covered by census and tagging and not included in the list of previous
beneficiaries) and the schedule of loan availment;
4. Project Profile;
5. Copies of Vicinity Maps, Topographic Maps (for properties with rolling terrain), Lot Plans and Technical Description duly signed by Geodetic Engineer and Schematic Subdivision Plans of CMP projects;
6. Certified xerox copies of Transfer Certificate of Title (TCT) and two (2) back TCTs of CMP projects;
7. Copy of Tax Declaration;
8. Copies of LGU Appraisal Reports of CMP projects;
9. Photos of Project/s particularly on the ff: and;
9.1 Access Road
9.2 Entrance
9.3 Alleys/Roads - w/housing units
9.4 Basic Utilities
10. Warranty Undertaking to be issued by the partner-LGU that:
10.1 The Community Association is a bonafide one and its acts are the voluntary acts of the members thereof for
the purpose of acquiring and keeping the property for themselves;
10.2 The member-beneficiaries have not availed of any government housing program;
10.3 The physical existence of the identified project/s have been established. The Title is authentic and is free from
liens and encumbrances; and
10.4 Deed of Negative Pledge with an undertaking that in the event the project/s are enrolled in the OCL, the said
project/s may not be sold/leased/encumbered without SHFC’s consent.
II. APPROVAL OF LOAN
a.Upon evaluation of documents and appropriate validation of the partner-LGUs compliance to LCMP criteria, SHFC shall present a report for approval by the Credit Committee (Crecom).
b.The partner-LGU shall be formally informed of the OCL approval by the Crecom thru a letter signed by the SHFC President.
III. AVAILMENT OF THE APPROVED OCL
The partner-LGU shall process prioritized projects strictly adhering to LCMP guidelines and consistent with the submitted project inventory and scheduled line availments. It shall accredit local housing partners (such as NGOs, POs, etc.) to act as project originators/program partners.
IV. RELEASE OF LOAN PROCEEDS
Upon completion of the full LCMP processing cycle for each project, the partner-LGU shall submit the loan documents required under the CMP guidelines to SHFC in six (6) sets (COA, PAEMD,TSD, LED, MED and LCMP Department).
V. PROCESSING TIME
The processing time for the release of loan proceeds shall not exceed sixty (60) days from receipt of the complete set of LCMP documents.
